Medical Case Management

Medical Case Managers help HIV-positive clients manage their health by overcoming barriers to care and adhering to prescribed treatment regimens.

Positive Connections transitioned to a medical case management model in late 2010. Medical case management requires specific activities that are not typically a part of traditional case management. These activities include: reviewing health insurance options and assisting with enrollment into health insurance (including Ryan White Part B if no other insurance is available,) reviewing eligibility every six months, coordinating medical care (between the client, physicians, mental health, substance abuse and dental providers), case consults with the primary HIV physician at least twice per year, reviewing medication and appointment adherence, conducting health education/risk reduction sessions, conducting a Substance Abuse Mental Illness Symptom screen, conducting acuity assessments every six months and maintaining ongoing contact to ensure the client is making progress on goals.

In addition to implementing these required activities, Positive Connections' medical case managers provide services that are not required, but are a part of a holistic approach to case management. These activities include: emergency financial assistance with prescription co-pays, utilities, rent or similar needs; providing Boost/Ensure or other high protein dietary supplements as recommended by the HIV physician; hosting support groups; assistance with housing through Shelter Plus Care and Kansas Care Though Housing; a food pantry specifically designed to offer high-protein foods to the most needy clients; and providing bus tickets and transportation to attend doctor appointments, lab appointments or address other HIV-related issues.

Positive Connections' four full time Medical Case Managers currently provide services to nearly 300 clients living with HIV.
